A RCA is a good investment as it finds the combined weaknesses of the design, the materials, the production of the components, the running and the management of the equipment.
A well performed RCA avoids creating a blame game. It shall instead be a positive and constructive process that finds the real causes and suggests improvements.
An issue, or a failure, can in this way become an important step to much improved reliability.
